Senior Support Advisors is a subscriber program that provides direct access to healthcare professionals, providing practical assistance for our older adults, families or caregivers.
The services of our pre-selected and credentialed professionals range from taking care of simple practical needs, overcoming environmental changes to providing the highest level of expert Geriatric Care Management.
It is the goal and desire for all of these services to be easily accessible by calling our 24 hour Call Center and by doing so, maintaining and preserving the best possible quality of life, in the most independent setting.
The subscriber program advocates for all its’ members receiving medical care.
For example, when a member is hospitalized, we will select and make the appropriate referral to our professional Provider Network and an advocate can be dispatched to the hospital, making sure you receive all necessary care.
In addition and if requested, we will provide the hospital with patients’ medical records and advanced directives information, as well as keeping designated family members up-to-date on important medical developments.
General Care Management
The General Care Management service is provided through a “referral” to selected Geriatric Care Managers within our network who will provide an array of options for our members and subsequent family members as they navigate and determine the course of action for the immediate needs, but also to establish short and long-term goals for on-going care, the Geriatric Care Manager will be appointed as needed and referred to the client or family member.
Daily tasks such as making sure that patients’ important bills are paid while they are in the hospital or assisting in the explanation and navigation of their insurance claims..again, .
We can match members through our designated Provider Network with Concierge Companions who share their interests and can address their particular needs. A client who loves the arts, for example, might be paired with an arts aficionado who can take him/her to nearby exhibits.
